Dedicated nurses create strong relationships and deliver personalized care to improve outcomes and mitigate costs. A typical range of savings is $0.90–$4 per member per month.2
Support health with remote monitoring
Remote monitoring is available for the heart failure and COPD programs. The individual weighs in twice a day and responds to questions about symptoms. Information is sent by phone to the nurse, who reviews it and responds proactively.
