HouseCalls supports members and health plans
Undiagnosed health conditions may lead to poor outcomes for members and costly outcomes for health plans. HouseCalls can aid in:
- Gaining a broader understanding of a member’s care needs
- Decreasing the risk of serious health events
- Prevent unnecessary hospital admissions
During the visit members benefit from:
- 45 to 60 minutes of one-on-one time with an Advanced Practice Clinician in the convenience of their own home
- A head-to-toe exam, medication reconciliation and appropriate health screenings
- Educational materials and resources tailored to their health care needs
