New West Physicians and Mountain View Medical Group are a part of Optum Colorado.  Some of our offices are now combined. You’ll find the same trusted providers under a new name.

To transfer your medical records to or from Optum Colorado, you must fill out a medical release form. If you are transferring medical records to Optum Colorado, please complete the form and bring it to your doctor's office upon completion. View and download the necessary forms here.

Your MyChart account gives parents, caregivers and legal guardians proxy access to their child’s health information. With proxy access you can do almost anything you can with your own personal account, with the following limitations based on the child’s age:

Ages 12 and under

Parents, caregivers and legal guardians can:

  • Pay bills
  • Schedule appointments, including well-child checks
  • Send messages to their child's doctors and health care providers
  • View immunization records, test results and more
  • View upcoming and past appointments

Ages 13–17

Once a child turns 13, there are some changes to what parents, caregivers and legal guardians can see in a child's MyChart account.  

You can still:  

  • Pay bills
  • View immunization records.  

However, to protect the child’s privacy, sensitive information related to appointments, test results and medications are no longer available. This offers teens a safe space to seek medical care and guidance.

Ages 18 and older

Once a child turns 18, parents, caregivers and legal guardians no longer have access to their child's health record in MyChart. You will have to be granted access by the adult child.  

A person 18 years old or older can choose to grant access to another person from their own MyChart account by following these steps:  

  1. Go to "Share MyRecord Activity"  
  2. Select "Friends and Family Access"  
  3. Click "Invite Someone"

*For life-threatening and time-sensitive injuries and illnesses, patients should call 911 or go to the nearest emergency room. DispatchHealth shouldn’t be used in a life-threatening emergency and doesn’t replace your primary care provider.
